Medication Forms

Any and all medication taken at school must have a written order by a physician or nurse practitioner. 

Rules for taking medications at school must be strictly adhered to as prescribed by the Hamilton Township BOE.   

The medication must be labeled with the child's name and must be in the original container. 

No loose medications in baggies or envelopes will be accepted.
